Information on what to include in an appendix and how to format it can be found in Section 2.14 of the APA manual (pp. 41-42).
The manual states that each appendix should begin on a separate page at the end of the paper (i.e., after the references, footnotes, tables, etc.). All appendices require a label (e.g., "Appendix", "Appendix A") and a title which should be bolded and centred on separate lines at the top of the page (p. 41).
