In Bloomberg use the code BCY to get to the Bankruptcy Dashboard.

In OECD.Stat, under the Data by theme menu see Industry and Services > Enterprise Statistics > Timely Indicators of Entrepreneurship > Bankruptcies of enterprises.

Use Capital IQ and hover over the Screening link at the top of the page and then click on Companies. In section for Corporate Actions select Bankruptcies

For the United States:
